CSS Children’s Special Services – Helps to meet the medical and financial needs of children birth to age 21 years with chronic illnesses. These may include orthopedic, heart, epilepsy, diabetes, renal, hearing loss, cleft lip/palate, and many other chronic or handicapping conditions. Community Development – This program is an active community based health council in Hawkins County, comprised of representatives in education, industry, health care, and local citizens working together for the health of all people in the community, identifying concerns, and developing plans to address these concerns. Dental Services – Provides services for children ages 0 to 21, such as fillings, cleaning, extractions, x-rays and caps. Arrangements through local schools allow children to be seen at the dental clinic during school hours, as transportation is provided by the school system. Epidemiology Services - Surveillance, testing, diagnosis, and treatment for various communicable diseases such as tuberculosis, sexually transmitted diseases and other mandated reportable diseases. Environmental Services – The Department of Environment and Conservation includes the division of groundwater protection, which oversees septic system planning and inspection, water sample testing and approval of subdivisions. The Department of General Environmental Health is responsible for food and sanitation inspection, rabies and insect vector control. Family Planning Services – The clinic provides consultation and medical assistance for all areas of reproductive health. This includes birth control for both males and females. Examinations include pelvic exam, breast exam, pap test, pregnancy testing, sexually transmitted disease testing and general tests such as blood pressure, urine and hemoglobin.  Health Educations Services – Health care information for the public is provided through the local media. Program presentations are provided to schools, churches, and community organizations upon their request. Free pamphlets and use of audiovisual materials are available for loan.Immunization Services – Provided for ages birth through adult. Tennessee Home Visiting - (two branches) HUGS (Help Us Grow Successfully) and CHAD (Child Health and Development) – Both programs provide home visiting/care coordination services at no cost to pregnant/post partum women, infants and young children. The care coordinator is a health care professional who provides services to address both the health and developmental needs of mothers and young children. WIC (Women, Infants, and Children) Nutritional Services – Supplemental food program for eligible infants, children and pregnant or breastfeeding women. Nutritional education and counseling are provided to individuals or groups. Best Start – Promotes breastfeeding through counseling, education and support for new mothers. School Health – assists the school system to ensure entry requirements are met by providing school entry physicals and immunizations. SIDS (Sudden Infant Death Syndrome) – Offer supportive and educational services for families who have had a death of a child from SIDS. Birth Certificates – Certified copies of birth certificates are issued for persons born in Tennessee since 1949. There is a charge for the search and one copy, with an additional charge for additional copies. Applications for births occurring prior to 1949 are available for the applicant to complete and mail to Nashville for a certified copy. Death Certificates – Certified copies are available for death certificates within the past 3 years. TENNderCare - EPSD&T (Early Periodic Screening, Diagnosis, and Treatments) – these screenings are provided for TennCare patients to age 21 and include (per the AAP periodicity table) a comprehensive health history and developmental assessment, comprehensive physical exam, vision screening, hearing screening, laboratory testing, immunizations and health education/anticipatory guidance. Paternity Acknowledgment – increases the number of legal paternity establishments for children born to unmarried parents. Primary Care – this service is available in the Church Hill office only. Basic health care services are provided to uninsured patients by a nurse practitioner or physician. These services include: • examination, diagnosis and treatment of acute, episodic illness • management of chronic disease • referral, coordinated assistance for medical specialists and medication assistance programs • limited pharmacy formulary • medically necessary laboratory testing Fee Sturcture and Appointments A fee may be required for some services. Charges are based on the Tennessee Department of Health Fee-for-Services policy. Charges may be adjusted based on family income. TennCare, Medicare and private insurances are accepted. All patients are required to have an appointment and verbal proof of income. WIC requires written documentation of income. All services are strictly confidential. Interpreter services are available.
